1. Form 1 is the "Academic Resume."
1) For degrees, please write the name of the degree followed by the awarding institution in parentheses as follows: "Doctor of Veterniary Medicine (Kitasato University)." Below this, please state your field of specialization.
2) For your education history, please begin with your high school.
3) Please provide details of degrees, veterinary licenses (with numbers), etc., in the "Qualifications" field.
4) Please print out on A4 and affix your seal (if you have one) and signature.
2. Form 2 is "Publications and Presentations."
1) Please read the PDF guide about how to prepare the "Publications and Presentations" form and follow the examples when completing.
