How to apply by post

Please note that the documents and fees required and the number of days required to issue a certificate in English differ from those required for a certificate in Japanese.

For Japanese certificates (about 1 week to issue)

1. A copy of the applicant's identity card
   The identity card must be one of the following. Copies of identity documents submitted will not be returned.
   (1) Driver's license
   (2) Passport
   (3) Health insurance card (*)
   (4) Alien Registration Card
   *If you use your health insurance card as an ID card, please submit a copy with the "insurer number" and "insured person etc. symbol/number" masked.

2. Application for a certificate
3. 証明書代
   郵便局で定額小為替を購入してください。（為替証書には何も記入しないでください。）
   卒業証明書、修了証明書、成績証明書、退学証明書   1通    200円
   特別な様式の証明書（調査書、在籍証明書、学位取得証明書、単位修得証明書等） 1通  1,000円
4. 返信用封筒・切手
   郵便番号、住所、氏名を記載し、切手を貼付してください。
   返信先が本人以外の場合は、 委任状が必要となります。
   ※返信用切手
   定　形 110円
   定形外 140円
   速達料金 + 300円

For English certificates (about 2-3 weeks to issue)

   Please purchase a money order from a post office. (Please do not write anything on the money order.)
   All 1,000 yen per certificate
5. 返信用封筒・切手

How to apply at the counter

You can apply for a certificate by filling in the application form and purchasing and affixing a certificate.
You will receive your certificate within 3 days of receipt. (except Saturdays, Sundays and public holidays)
You can also receive your certificate by post. Please bring a self-addressed stamped envelope with you when you apply.
Please make sure you bring one of the following forms of identification with you when you apply and collect your card.
(1) Driver's license
(2) Passport
(3) Health insurance card (*)
(4) Alien Registration Card
*If you use your health insurance card as your ID, please submit a copy with the "insurer number" and "insured person etc. symbol/number" masked.

If you are a proxy, please bring the following three items
(1) A copy of your identity card
(2) A letter of proxy
(3) Identity card of your representative

Notes

If you change your family name or domicile (prefecture only) after graduation, you will need to submit a certificate of personal details (an extract from your family register) [original copy].
